Outreach Recovery Program
For adults aged 18 – 65 diagnosed with mental illness. We provide the support they need to live in their own home or place of choice, while enabling them to take advantage of our care-coordination services. The aim of the program is to support participants to identify ways their mental health could be improved using our recovery model, to maintain their community living choices and also engage them in other services of their choice.
How we can help
Our outreach recovery service is a one-on-one support program that aims to support participants to live in their own home or place of choice while connecting them with relevant services in their community. This service is available in the South and on the North West Coast (from Deloraine all the way through to Smithton).
Program services include:
- Receive individualised, one-on-one support
- Take part either in your own home or place of choice
- Develop life skills and find new ways to do things
- Develop coping strategies
- Work on specific recovery goals
