Richmond Futures requires detailed information about potential SIL participants in order to determine suitability and to ensure we are able to meet individual needs. Additional and/or clarifying information will be requested as needed.
The checklist below draws on two assessment tools commonly used to capture a person’s level of ability to self-care (Instrumental Activities of Daily Living Scale, Lawton and Index of Activities of Daily Living, Barthel).
When completing this checklist please consider what the participant can do and mark the most appropriate response. Additional information can be attached if necessary.
